Does cold water immersion help stress and mood?

The verdict
Weaker than the enthusiasm suggests. Across 11 studies and 3,177 people, stress fell significantly only at 12 hours after immersion, with nothing at any other timepoint, and mood showed no significant difference against passive recovery. Inflammation rose immediately afterwards. Sleep quality improved in limited studies. The rush you feel is real, and it is not the same as a measured benefit.

What does cold water actually do to the body?
Immersion in cold water produces an immediate and dramatic response. Heart rate and blood pressure rise, breathing quickens, oxygen uptake increases, and the body releases noradrenaline and cortisol. This is a strong sympathetic activation, which is the opposite of the calm state people usually describe afterwards.
The feeling of clarity and elation that follows is real and worth naming honestly. It is what a sharp stress response feels like from the inside, and the pleasant part arrives as it subsides.
Does it reduce stress?
Only at one specific point, on the best available evidence. A 2025 systematic review and meta-analysis of 11 studies covering 3,177 people found a significant reduction in stress 12 hours after immersion. There was no effect immediately, at one hour, at 24 hours, or at 48 hours.
Cain and colleagues published this in PLoS One. A benefit that appears at 12 hours and is absent at 24 is a thin and slightly odd result, and it is what the pooled data shows.
The mood finding is the one most people will find surprising. There was no significant difference in mood between cold water immersion and passive recovery. The uplift is genuine as an experience, and it does not separate from a control group on the measures used.
What else did the review find?
Sleep quality improved after immersion, though from a small number of studies with mostly male participants. Immunity showed no significant change immediately or at one hour by meta-analysis, while narrative evidence pointed to longer-term effects including a 29 per cent reduction in sickness absence among cold shower users.
One finding deserves attention because it cuts against the usual framing. Inflammation increased significantly immediately after immersion and at one hour. Cold water is widely promoted as anti-inflammatory, and the acute response measured here goes the other way.
The authors' limitations are substantial: small samples, few randomised trials, very little representation of women, mostly single sessions, and protocols that vary widely in temperature and duration.
Why does it feel so effective?
Because doing something hard on purpose, first thing, changes how the rest of the day feels. Cold exposure is a small voluntary hardship you complete, and completing it carries a sense of capability that has nothing to do with water temperature.
Context matters too. The review authors note that a study finding acute mood improvement had people entering the sea together in a group, which makes it difficult to separate the cold from the company and the outdoors.
None of that makes the practice worthless. It relocates the explanation from physiology to something more ordinary and more reliable.
Is it safe, and is it worth doing?
The cold shock response is a genuine cardiac event, and people with heart conditions, high blood pressure, or a history of arrhythmia should speak to a clinician first. Never enter cold open water alone, and never combine it with breath holding, which is how experienced swimmers drown.
If you enjoy it, keep going. The honest basis is that you like it and it starts your day with something completed, and that is a reasonable basis.
If you are doing it purely for stress or mood, the evidence is thin, and slow breathing has better support at no risk. If the goal is recovery from exercise, that is a separate and better-studied question, and immediate cold exposure appears to blunt some training adaptations.

- Are cold showers as good as ice baths?
- The sickness absence finding came from cold shower studies, and the two have not been compared head to head on stress or mood.
