Which brain health supplements have real evidence behind them?

The verdict
Two hold up modestly. A daily multivitamin produced a small memory and cognition benefit across more than 5,000 people, and creatine shows a small effect on memory. Two of the most popular have been properly tested and failed: omega-3 produced no cognitive benefit in a 4,218 person trial, and ginkgo produced none across 3,069 people followed for six years.

Do brain health supplements work?
Mostly they have been tested less than the packaging implies, and where large trials exist the results are often null. Two exceptions have modest support: a daily multivitamin and creatine. Both produce small effects. Neither approaches what the category advertises.
The useful thing about this question is that several of the most popular ingredients have now been through large, long, placebo-controlled trials. That is rare in supplements, and it means you can get real answers instead of mechanism stories.
Below, each ingredient with a serious trial behind it, and what that trial found.
Does a daily multivitamin help memory?
The best evidence says yes, by a small margin. Pooling three studies inside the COSMOS trial, covering more than 5,000 older adults, daily multivitamin use improved global cognition by 0.07 standard deviations and episodic memory by 0.06. The authors describe the size as roughly two years of cognitive aging.
Vyas and colleagues published this in the American Journal of Clinical Nutrition in 2024. The design is strong: randomised, double blind, placebo controlled, two years of follow-up, with the meta-analysis drawing on three separate cognitive substudies.
Two things belong beside the finding. Mars Edge, a commercial nutrition business, funded the work through an investigator-initiated grant and donated the study pills, and several authors report support from Mars Edge and similar bodies. The trial also used one specific formulation, so the result does not transfer automatically to whatever is on the shelf.
The authors also note the participants were mostly white and comparatively affluent, and that they could not identify which vitamins or minerals produced the effect.
Does creatine do anything for the brain?
A little, for memory. A 2024 meta-analysis of 16 randomised trials covering 492 adults found a small significant improvement in memory, along with faster attention and processing speed tasks. Executive function showed no effect. The clearest gains appeared in older adults.
Xu and colleagues published this in Frontiers in Nutrition. The memory effect came out at SMD 0.31, which is small and consistent. Processing speed improved, with the authors noting the effect was more marked among women.
The trials varied widely in design, sample size and testing method, and most enrolled healthy adults, so evidence for specific groups stays thin. Creatine is also better studied for muscle than for cognition, and the brain work is younger and smaller.
A pattern worth noting across this literature: effects tend to be larger in people who started with less, whether that is lower baseline creatine, poorer diet, or older age. Supplementing a system already well supplied does less.
What about omega-3 and ginkgo?
Both have been tested well and both came back null for cognition. Omega-3 at 1 g daily produced no cognitive benefit across 4,218 older adults over two to three years. Ginkgo produced no slowing of decline across 3,069 people followed for a median of six years.
The omega-3 result comes from the VITAL trial, reported by Kang and colleagues in 2022. The pooled difference in annual cognitive decline was effectively zero. The authors are careful to say the group was well educated, declined little over the period, and had few people with low omega-3 intake, so a modest effect in a different population could have been missed.
The ginkgo result comes from the Ginkgo Evaluation of Memory study, published by Snitz and colleagues in JAMA in 2009. Rates of decline were the same as placebo across memory, attention, language, visuospatial ability and executive function. Six years, 3,069 people, no signal anywhere.
Ginkgo is the clearest case in this whole category. It has been sold for decades for memory, it has been tested properly, and it did nothing.
What would help more than any of this?
Sleep, movement, and relationships all have larger and better-established effects on cognition than anything in a supplement aisle. The multivitamin finding of roughly two years of cognitive aging is genuine, and it is small next to what those three carry.
Social connection is the strongest single comparison available. The mortality evidence there is large, replicated, and graded evidence-based on this site.
If you want a reasonable position: a supplement is worth considering where you have a shortfall to correct, and it is a poor substitute for the things that move the needle. Speak to a clinician who knows your history before starting anything, particularly if you take medication.
Questions people ask
- Should I take a multivitamin for my brain?
- The evidence supports a small benefit, from one formulation, in a trial funded by a nutrition company. That is a reasonable basis for a low-cost choice and a weak basis for expecting much.
- Why do supplement websites cite so many studies?
- Most cite mechanism work, cell studies, animal work, or small short trials. The question that matters is whether a large randomised trial in people found a benefit, and for most ingredients that trial has never run.
- What about lion’s mane, magnesium L-threonate, or nootropic blends?
- None has trial evidence approaching the scale of the studies above. They sit at insufficient evidence, which describes what is known and makes no claim they do nothing.
- Is any of this dangerous?
- Supplements interact with medication and some carry real risks at high doses. Anything you take regularly belongs in a conversation with your doctor or pharmacist.
